PET/CT WITH CONTRAST ENHANCED DIAGNOSTIC CT SCAN OF CHEST ABDOMEN AND PELVIS FOR SQUAMOUS CELL CARCINOMA OF THE LIP - 02/13/12 16:30:00 HISTORY: Squamous cell carcinoma of the lip with prior surgery and radiation therapy DATE OF DICTATION: 02/14/12 TECHNIQUE: Following the administration of approximately 13.34 mCi of FDG 18 via the right antecubital fossa with a concurrent blood sugar level of 86 mg/dl as well as the oral administration of Gastrografin and intravenous administration of Optiray 320 a 64 minute delayed PET/CT is acquired from skull base to mid thigh. This is a follow-up scan for this patient for this tumor type. COMPARISON: A similarly performed study dated May 10, 2011 is available for review. FINDINGS: PET SCAN: Neck:There is bilateral tonsillar prominence with increased uptake which may merely represent inflammatory changes within the tonsils. This is similar to the prior study. The previously present submandibular lymphadenopathy has resolved except for a small focus of SUV of 3.5 in the right cheek. A definitive focus however is not seen on CT. There is still left level 3 lymphadenopathy with a maximum SUV of 4.6. Chest:Only physiologic distribution of FDG is present. Abdomen:Only physiologic distribution of FDG is present. Pelvis:Only physiologic distribution of FDG is present. Other: CT SCAN: Neck:There is bilateral maxillary sinusitis. Patient has previously undergone extensive resection of the upper lip, portions of the anterior hard palate and extraction of all maxillary teeth. The patient appears to have undergone left neck surgery as well. There are lateral shotty submandibular lymph nodes. A borderline lower left level 3 lymph node is also present which demonstrates increased uptake by PET. There are no infratemporal fossa masses or parapharyngeal masses. Chest:There are no parenchymal nodules, pleural effusions or infiltrates. There is no mediastinal or axillary lymphadenopathy. Abdomen:The liver, spleen, adrenal glands, kidneys and pancreas reveal no focal findings. The gallbladder is unremarkable. There is no retroperitoneal lymphadenopathy. Pelvis:There is no pelvic lymphadenopathy. A few shotty inguinal lymph nodes are present. The prostate is not enlarged. Other: IMPRESSION: Evidence of prior surgical resection of upper lip neoplasm with residual or recurrent tumor in cervical lymph node on the left and possibly in adjacent right cheek even in the absence of a CT correlates to area of increased activity on the right. I don’t understand how so many people get a hold of these reports with not explanation from the doctor who ordered it. IT IS THEIR JOB TO EXPLAIN IT TO YOU AND ANSWER YOUR QUESTIONS. Clearly you have been through pretty extensive surgery and radiation. There is still some cancer in your lymph nodes and possibly in your right cheek.
